Revised Sept. 22, 1997

Several of these spread sheets make use of Lotus123 macros. I knew very little about Lotus123 prior to purchasing my palmtop and the palmtop manual did little to change this fact. I found an excellent book on the subject which I recommend to anyone:
  Understanding 1-2-3
  Release 2.3 & 2.4 for DOS
  Second edition
  by Rebecca Bridges Altman
  From Sybex publishers (1-800-227-2346)

You need to have all of these files in Lotus123's default directory (Menu Worksheet Global Default Directory). The default is C:\_dat unless you have changed it. Set automatic execution of macros to ON (Menu Worksheet Global Default Autoexec).

00000000.WK1: this is a blank spreadsheet. The name ensures that it will always be first in an alphabetical listing of files. When AUTO123.WK1 (see below) displays a list of files to open, choose this one if you just want to work on a blank spreadsheet. Remember to give your work a new name if you add anything so you will always have a blank spreadsheet waiting at the top of your file list.

AMINOGLY.WK1: calculates doses for aminoglycoside antibiotics. The formulas were taken from a handout prepared by the PharmD of my residency program. I converted them to spreadsheet form myself. They seem to give reasonable results, but be sure to use you judgement when deciding whether to rely on these numbers. Use ZOOM to see more of the spreadsheet. Enter information about the patient in the left-hand column. If you are not sure, leave the default value. The calculated loading dose and subsequent dose as well as the interval are shown in E1-E3. The predicted peak and trough with those doses are shown in H7&8. If you want to use a different dose or interval, enter them in E4 & E5 and the predicted values will be changed to reflect this info (enter zero to make the predicted values based on the dose & interval in E2 & E3 again). When you have measured peak and trough values (do around the fourth dose), you can refigure your doses. Enter the Peak, Trough, Time from trough to infusion (in hours) and Time from infusion to peak (in hours). A new dose and interval will be calculated and shown in E21-E22. As a bonus, you get a very good estimate of the patient's Creatinine Clearence in H11. If you want to see what other doses will do to the peak and trough, enter the info in A 21-22 and read the result from E15-16

ANTIBIO.WK1: calculates doses of antibiotics. I designed it for pediatric doses, but it will give adult doses too. This spreadsheet uses a series of macros. You can select an item by holding down the ALT key and typing its first letter. For example pressing ALT and Q will select Quit and end the program and shut down Lotus123 (the spreadsheet is automatically saved each time you quit).
You can SEARCH for a drug by typing in either the trade or generic name but the generic name is always shown in the results screen. You can enter just part of the name for a search, ie AUGME with find Augmentin (amoxicillin-clavulenate).
You can enter the child's WEIGHT in either pounds or kilograms but the results always shows Kg.
If the drug is not found, you will see ERR for the drug name. Try again with a different name or check your spelling (my spelling is not garenteed).
You have to select VIEW and then choose Suspension, Pills, or IM/IV to see your results. The view uses a Lotus Menu. You can select by pressing the first letter of an item or by highlighing it with the arrow keys and pressing ENTER. ESC will exit the menu.
You can add NEW drugs as well. I designed the new drug input to make it easy to add drugs from the listing in the Harriet Lane Handbook. Enter zero or a space for any lines you don't fill in (ie if there is only one suspension strength, enter 0 when asked for the others). If you make a mistake or for some reason don't want your new information saved then hit CTRL-BREAK then ESC then Menu Quit and answer Yes to the "Quit anyway" question.

AUTO123.WK1: Lotus123 always loads a file with this name and runs the macro in it whenever you start up (ie when you push the 123 blue key). This macro puts up a list of all your spreadsheets. You can move the cursor around with the arrow keys and press ENTER to load the file you want.

CODE.WK1: calculates pediatric doses of emergency medications. Type in the child's weight in Kg in the cell under the cursor and then move down to see all of the doses. DO NOT save your changes so the cursor will always start on the correct cell. Use ZOOM to make the comments visable.

DRIPS.WK1: calculates how many cc/hr to run a drip to give a specified dose in mcg/kg/min. Fill in A1-4 with the desired dose, the concentration of the drug, and pt's weight in Kg. Common dose ranges are shown as well.

QT.WK1: displays a graph of normal QT intervals for various heart rates. Press ESC to quit viewing the graph.
